They will normally just send you a duplicate form, with a replied paid envelope, fill out the form and send it back!! It sounds like you were on a 12 week lodgement cycle? and should have been told to lodge that last form to complete the cycle! Once the form is returned, it gets processed and the debt is USUALLY wiped!
